Job Description

1. Community Mobilisation and Outreach

  • Conduct home-to-home visits to identify and engage pregnant women, lactating mothers, children and individuals living with/at risk of NCDs.
  • Mobilise community members to participate in program activities, health camps, and awareness sessions.
  • Facilitate community meetings, group discussions, and awareness sessions on health, nutrition, and
  • Use IEC materials during household visits and community meetings to promote healthy practices.
  • Support campaigns related to health days, immunisation drives, and government health initiatives.
  • Conduct screening for NCD in the community.

2. Community Linkages with Government Services

  • Facilitate linkages between community members and government health services such as Anganwadi Centres, Primary Health Centres, Hospital and public health programs.
  • Support beneficiaries in accessing government schemes related to health, nutrition, and social welfare.
  • Coordinate and collaborate with frontline workers such as ASHA, Anganwadi Workers, and ANMs.

3. Data Collection and Documentation

  • Maintain records of household visits, beneficiaries reached, and community activities conducted.
  • Assist in collecting field data related to program indicators.
  • Ensure timely reporting and documentation of community-level activities.

4. Community Network Building

  • Build strong relationships with community leaders, local groups and stakeholders.
  • Support the formation and strengthening of mothers’ groups, and community health committees.
  • Encourage community participation and ownership of program activities.

5. Support Program Implementation

  • Assist the program team in organising training, workshops, health camps, and outreach activities.
  • Participate in regular team meetings and share updates from the field.
  • Provide feedback on community needs and challenges to improve program implementation.
