Education:

  • Graduate in Social Work, Science or related field.
  • Diploma or certification in community health / public health will be an added advantage.

Experience:

  • 0-2 years of experience in community health or NGO field projects.
  • Experience working in Maternal & Child Health (MCH) and Non-Communicable Disease (NCD) programs desirable.
  • Experience in community mobilization and field team supervision will be preferred.

Skills:

  • Strong team supervision skills.
  • Good communication and community engagement abilities.
  • Knowledge of community health practices and public health programs.
  • Ability to work in field settings and travel within project areas.
  • Basic computer skills for reporting and documentation.

Job Description

1. Field Program Implementation

  • Supervise day-to-day implementation of MCH and NCD project activities in the community.
  • Ensure field staff conduct regular household visits, awareness sessions, and beneficiary follow-ups.
  • Support organisation of health camps, screening programs, and community meetings.

2. Team Supervision

  • Supervise and guide community health workers, volunteers, and outreach staff.
  • Provide on-ground support, mentoring, and performance monitoring of field staff.
  • Conduct regular field visits to ensure quality of service delivery.

3. Maternal & Child Health (MCH) Activities

  • Ensure identification and tracking of pregnant women, lactating mothers, and children.
  • Monitor antenatal and postnatal care follow-ups.
  • Promote immunization, nutrition, and safe motherhood practices in the community.
  • Facilitate referral of high-risk cases to the clinic or hospital.

4. NCD Activities

  • Support community-based screening for hypertension, diabetes, and other NCDs.
  • Ensure follow-up of patients diagnosed with NCDs.
  • Promote awareness on healthy lifestyle, diet, and physical activity.

5. Community Engagement

  • Build relationships with community leaders, local health workers, and government health facilities.
  • Mobilise community members to participate in health camps and awareness programs.
  • Address community concerns related to health services.

6. Monitoring, Documentation & Reporting

  • Maintain field activity records, beneficiary data, and reports.
  • Compile and submit weekly and monthly reports to the Project Coordinator.
  • Ensure accuracy of field data and proper documentation.

7. Coordination

  • Coordinate with clinic staff, medical officers, and project team members.
  • Support implementation of government health initiatives related to MCH and NCD.
